You may call your VISA or MASTERCARD into us at 1-800-441-5571.
You may send a check to 429 E. Dupont Road #98, Fort Wayne IN 46825.
You may have it sent COD via UPS for an extra $11 as of 1/1/12.
SHIPPING:
We ship UPS primarily so we need a physical address, not PO Box. If the address is residential, an additional fee is added. On occasion, we will use USPS. We usually ship all orders within 1 to 2 days from Indiana. Please expect shipment(s) to arrive within 2 to 5 business days unless something is on backorder.
SALES TAX:
7% INDIANA sales tax must be and will be collected from doctors who reside in and work in IN unless an INDIANA Sales Tax EXEMPT form is on file with us.
